If you are a woman surviving any form of cancer, you are eligible for this free service as long as your support group, hospital, church or synagogue is an organization which can refer its members to Shop Well with You. Contact the support organization with which you participate to find out if they have a relationship with Shop Well with You.

If the support organization has not yet become a referring organization, send them the Shop Well with You website so that they can easily get involved. If your support organization is already involved with Shop Well with You, fill out a referral form that they will provide for you. This form will specify your particular needs so that Shop Well with You can identify the most comprehensive service for you.
